General Description of Required Documentation
|Address Change||Submit request in writing with your signature.
Change of Address Form
|Name Change||Marriage Certificate, Divorce Decree reinstating maiden name or other legal documentation of name change.|
|Company Name Change||Certificate of Name Change, including W-9 form.|
|Company Name Change Due to Merger||Certificate of Merger, including W-9 form.|
|Appointment of Attorney-In-Fact||Copy of Power of Attorney.|
|Transfer or Sale of Interest||Conveyance filed of record in the County where the property is located and W-9 form(s).|
|Ownership Change Due to Divorce||Divorce Decree, including Settlement Agreement and recorded conveyances (if appropriate) with W-9 form(s) for new owner (if applicable).|
Trust or Partnerships
Trust or Partnership is Terminated
Trust or Partnership Agreement and the document conveying the interest in a property to a Trust or Partnership recorded in the County where the property is located. W-9 form(s) for the Trust if not using owner's social security number.
Dissolution of Trust or Partnership. If appropriate, recorded conveyance to the beneficiary, including W-9 form(s) for any beneficiary.
|Changes in Trustee||Document showing change of Trustee such as acceptance of Successor Trustee.|
|Joint Tenancy with Death of One Tenant||Death Certificate of Deceased Tenant and W-9 form.|
Testate - Owner died with a Will and probate proceedings are to be conducted in the state where the property is located.
Testate - Owner died with a Will and probate proceedings are to be conducted in a state other than the state where the property is located.
Intestate - Owner dies without a Will and the Estate will not be probated. Laws of Descent and Distribution will apply as relevant.
Death Certificate, Last Will and Testament, Order Admitting Will to Probate, Letters Testamentary or Letters of Administration, Final Decree of Distribution or Judgment of Possession (if applicable), listing of current mailing addresses for heirs and W-9 form(s).
Death Certificate, Last Will and Testament, Order Admitting Will to Probate, Letters Testamentary or Letters of Administration, Final Decree of Distribution or Judgment of Possession (if applicable), Documentation of Ancillary Probate Proceedings opened in all affected states and recorded in the County where the property is located or Proof of Death and Heirship Affidavit and W-9 form(s).
Death Certificate, Proof of Death and Heirship Affidavit prepared by disinterested party, executed before a notary public, recorded in the County where the property is located and W-9 form(s).

|Life Estate - Death||Life Tenant's Death Certificate Names and Addresses of Remaindermen and W-9 form(s).|
Plan of Reorganization Order confirming Plan of Reorganization Recorded Conveyance (as appropriate), Order Naming Trustee and W-9 form(s).
